Booking/Shift notes are legal documents used for NDIS compliance and care continuity.
- Be Objective: Record facts, not opinions. Instead of “The member was angry,” write “The member raised their voice and refused to eat lunch.”
- Link to Goals: Mention how the shift activities align with the member’s NDIS goals (e.g., “Practised social skills by ordering coffee at a cafe”).
- Be Specific: Include times, medications administered (if applicable), and any changes in health or mood.
- Professionalism: Use clear language and avoid slang or overly casual abbreviations.
