Documentation Required
All required documentation must be completed before the end of your shift.
- Blu Sky Care Incident Report
- Case notes — clear, factual, and objective
- Medication Error Form (if applicable)
Management Will Handle:
The manager or delegated supervisor will manage:
- NDIS Commission notifications
- Contact with family or nominee (unless this has been delegated to you)
Follow-Up Responsibilities
After the incident has been reported, carers may also be required to:
- Monitor the participant’s condition
- Attend a debrief if required
- Review risk management strategies
- Follow any updated instructions in the participant’s support plan
