How to Report Incidents?

Notify Immediately

As soon as it is safe to do so, contact:

  • Team Leader
  • On-Call Manager (after hours)

 

Documentation Required

All required documentation must be completed before the end of your shift.

  • Blu Sky Care Incident Report
  • Case notes — clear, factual, and objective
  • Medication Error Form (if applicable)

 

Management Will Handle:

The manager or delegated supervisor will manage:

  • NDIS Commission notifications
  • Contact with family or nominee (unless this has been delegated to you)

 

Follow-Up Responsibilities

After the incident has been reported, carers may also be required to:

  • Monitor the participant’s condition
  • Attend a debrief if required
  • Review risk management strategies
  • Follow any updated instructions in the participant’s support plan
